Please fill out the following form accurately before attending lessons. This waiver is only valid for 2024. The waiver must be filled out each year reardless of previous submissions.
We are so excited to swim with you this summer! Although we intend to have a very safe and enjoyable experience with every one of our students, there are always some risks that come with being in the water. Safety, including teachers being safe and teaching students how to be safe, is our first priority, but even still swimming can be dangerous and lead to property damage, bodily injury, permanent disability, and even death.
By signing this form, you are hereby agreeing to the following for yourself/and or children and/or grandchildren or other individuals you are considered a guardian for, as a result of any activity (includes but is not limited to swimming, swimming lessons, playing in or any surrounding areas or play structures) taking place at Lake Point Swim School (2045 Pheasant Lane, Lake Point, UT 84074):
· I understand that swimming is a dangerous activity and can lead to harmful or fateful consequences.
· I understand that if my child is not of good health or has some condition that impairs the child’s swimming capabilities or abilities to maintain safety in the water or property of Lake Point Swim School that they should not participate in any activities on the premises.
· I agree that I will not hold Lake Point Swim School or any instructors or other participants accountable for any kind of personal injury or harm that may occur at Lake Point Swim School.
· Any damage to private property by participants or observers will be repaired or compensated for by the guardian that signs the said form.
· I release any representative of Lake Point Swim School to perform or call for any medical emergency services (including but not limited to basic first aid, CPR, other EMT providers), furthermore, I agree to pay all costs associated with care and medical emergency services.
